Health Care Programs
Preventive care and disease management keeps people healthy and saves the state precious resources. DHS offers programs that provide care for the well-being of eligible Minnesotans. People who live in Minnesota may qualify for Minnesota Health Care Programs. You must meet program eligibility rules including income limits to qualify.

Personal Care Assistance
Personal care assistance services help a person with day-to-day activities in their home and community. PCAs help people with activities of daily living, health-related procedures and tasks, observation and redirection of behaviors and instrumental activities of daily living for adults.

School Health
Polk County Public Health contracts with school districts to provide pre-school screenings, hearing and vision screenings, school health services, health education and technical assistance with immunization review and requirements.

Child & Teen Checkups / Outreach
Child and Teen Check-Ups are well child visits for children ages birth through 20 eligible for Medical Assistance and Minnesota Care.
They include:
  • Vision and hearing check
  • Laboratory tests, including lead, blood and urine as needed
  • Immunizations if needed
  • A complete head to toe physical assessment
  • Health education and counseling about healthy lifestyles
  • Nutrition assessment
  • Developmental assessment

Follow Along Program
When do kids start talking and walking? The Follow Along Program is about child development. It lets you know if your child is playing, talking, growing, moving, and acting like other children the same age.

Polk County Public Health offers home visits to families with children under the age of 3 when there is an identified need.

Nurse Family Partnership
Nurse-Family Partnership is a free, voluntary program that partners first-time mothers with nurses. The goal is to have healthy pregnancies, improve child health and development, and become more economically sufficient.

Prenatal Education
A public health nurse can work with you one-on-one in the community or in your home to provide education for a healthy pregnancy. Education will include physical and emotional changes during pregnancy, how to prepare for labor and birth, breastfeeding, newborn care, and growth and development.

Post-partum/Newborn Visits
A public health nurse will visit you in the community after you return home with your baby. The nurse will assess baby’s growth and development, answer any questions you may have and will connect you to community resources available to you.

Child Passenger Car Seat Distribution & Education
Polk County Public Health provides education for choosing the right child safety restraint and instructions for correct use. Free car seats are available through Public Health to eligible Medical Assistance and MinnesotaCare members.

Dental Fluoride

Blood Lead Tests
Lead poisoning can cause learning, behavior and health problems in young children. Lead can cause high blood pressure and kidney damage in adults. Lead testing is available on Public Health days or by appointment. High lead case management available as needed.

Sexual Health
Sexual Health Nurse Practitioner on staff. Sexual Health clinic and walk-ins available. Provides:
  • Pregnancy testing and counseling
  • Birth control options (condoms, pills, Depo shot, Implant & IUD) & Sexual Health Education
  • Routine Gynecological Exams and Breast Exams
  • Cervical Caner Screening
  • Sexually Transmitted Infection Screening, diagnosis, and treatment (chlamydia, gonorrhea, syphilis, HIV, Hep C)
  • Emergency Contraception
  • Condoms

Help Me Grow
Program designed for children with special needs such as speech delays, movement problems or learning disabilities. Provide coordination of services such as physical therapy, speech therapy, early childhood special education or any other needed services to help children develop.

Child Immunizations
Immunizations are available in the office by appointment to children 0-18 who are eligible for the Minnesota Vaccines for Children Program. Eligibility includes those who are uninsured, have insurance through a state-funded plan or privately insured but vaccines are not covered.

Adult Immunizations
Immunizations are available in the office by appointment for adults 19 and older who are eligible for the Uninsured and Underinsured Adult Vaccines Program. Eligibility includes those who are uninsured or those who are insured but vaccines are not covered. Immunizations are also provided to adults on a state-funded insurance plan.

Women, Infants, and Children (WIC)
WIC is a special supplemental nutrition and breastfeeding program for Women, Infants and Children. It provides healthy food, nutrition education, counseling, health information, and referrals to health and other social services.

Supplemental Nutrition Assistance Program (SNAP)
SNAP, formerly known as Food Stamps, helps Minnesotans with low incomes get the food they need for nutritious and well-balanced meals. The program provides support to help stretch your household food budget. It is not intended to meet all of your household’s food needs. It is a supplement. If you are approved for SNAP, you can use your benefits at many stores, farmers markets and senior dining sites. Electronic Benefits Transfer (EBT) debit cards are used to make food purchases. Family Health
Family practice physicians provide basic health care to all members of a family. Appointments can be made for routine annual exams, well-child visits, or acute (short-term treatment for an injury or episode of illness) care.
