Case Management/Care Coordination
To coordinate Alternative Care, Elderly Waiver, Community Alternatives for Disabled Individuals, Brain Injury and Developmental Disability services. Assistance provided to eligible consumers that includes health risk assessment, coordinated care planning, connections to community services and identifies service providers.

The Alternative Care (AC) program provides home and community-based services to people who need nursing home level of care but choose to live in the community. The Alternative Care program provides many of the same services as the Elderly Waiver program. Alternative Care is for people with low income and assets who are not eligible for Medical Assistance. To qualify you must:
- Be age 65 or older
- Need nursing home level of care as determined by the Long-Term Care Consultation process
- Not have enough income and assets to pay for a nursing home stay lasting longer than 135 days
- Need services that Alternative Care can provide for less than 75 percent of what Medical Assistance would pay for an older person with a similar level of need
- Have no other way to pay for the services

Long Term Care Services
The Elderly Waiver (EW) program provides home and community-based services to people who need nursing home level of care but choose to live in the community. You must qualify for Medical Assistance to be eligible for Elderly Waiver services. To qualify you must:
- Be age 65 or older
- Be eligible for Medical Assistance
- Need nursing home level of care as determined by the Long-Term Care Consultation process
- Need services that Elderly Waiver can provide for less than the cost of care in a nursing home

Tri-County Community Corrections – Chore Service Program
The Chore Service Program provides outside chore services, such as raking leaves, snow removal, minor outside repairs, changing storm windows/screens, to individuals aged 60 or over. Referrals are initiated via the Senior Linkage Line at 1-800-333-2433.

Preventive care and disease management keeps people healthy and saves the state precious resources. DHS offers programs that provide care for the well-being of eligible Minnesotans. People who live in Minnesota may qualify for Minnesota Health Care Programs. You must meet program eligibility rules including income limits to qualify.

Minnesota Supplemental Aid - MSA
Minnesota Supplemental Aid (MSA) provides cash assistance to help adults who get Supplemental Security Income (SSI) pay for their basic needs. Some people who are blind, have a disability or are older than 65 but do not get SSI because their other income is too high may also be eligible for MSA if they meet the income limit. The RiverView Care Center is a 24 bed, skilled nursing facility that specializes in dementia care in Crookston. At RiverView Care Center we offer a locked facility, conveniently located as part of RiverView Health with internal access to hospital, clinic, radiology, laboratory, emergency department, rehab services, and pharmacy services.
